Adult Accommodations Assessment FAQs
What are accommodation assessments like?
We will meet online through Zoom. The initial assessment lasts up to two and a half hours, and we will take breaks as needed.
We can also split up the initial assessment over multiple days if needed. During the assessment we will have a conversation about your concerns, your history, potential accommodations, and anything else you think is important for us to know.
There is no test.
What if we do not cover everything in the assessment?
Prior to the assessment and after the assessment (before the final paperwork is signed), you can submit any information you think would be helpful for us to include in our report.
Often folx will type up a word document and upload it in the client portal or send via secure messaging.
Do I have to get reports from other providers?
Requesting accommodations requires first having a diagnosis. We will need proof of any diagnoses you have received from other providers so we can be as thorough as possible.
If you feel that it would be helpful to have others provide information or join you during the assessment, we welcome that!
When do I get the final paperwork (e.g., completed forms, letters)?
About two weeks after the initial assessment, we will schedule a follow-up session to review the final report and accommodation documentation.
This session lasts up to an hour.
Together we will go through the paperwork section-by-section, confirm the accommodations match your needs, and discuss next steps for support.
How do I know if an accommodations assessment will work for me?
If you believe having a formal assessment for accommodations from someone with expertise, then you have come to the right place.
What do I do with the final paperwork?
You get a copy of the paperwork sent through your patient portal.
You can share your copy with your work or school, or we are happy to send it to anyone you would like to receive it.
